Formerly, when immigration rules were not as strict, it was not unusual for Slavic family names to be altered  upon arrival in the USA.

Surname  examples:
Kostlivy is also spelled as Kostlevy
Feifarek (what I believe to be the original Czech spelling)  is also spelled as Fifarek, Fajfarek or Fiefarek

Czech feminine surnames are usually formed from the masculine version by adding the suffix 'vá' or 'ová'. 
For example,  Kostlivý is Kostlivá,  Janata is Janatová,  Suchý is Suchá   etc.
